“Together, these roles make B-12 essential for sharp thinking, memory, and mental energy,” Fotuhi stated. When B-12 levels are low, nerve signals may slow down or misfire, increasing the susceptibility of brain cells to damage. Prolonged deficiency can result in confusion, balance issues, slower cognitive processing, and trouble concentrating. Research has even linked higher homocysteine levels to cognitive decline and neurodegenerative diseases like Alzheimer’s and other forms of dementia.

Recognizing the Symptoms of a Deficiency

Individuals experiencing low vitamin B-12 levels may notice a range of symptoms affecting various body systems. Common signs include fatigue, weakness due to anemia, numbness or tingling in the extremities, heart palpitations, memory issues, difficulty concentrating, balance problems, and mood changes such as irritability and depression.

While anemia is often associated with B-12 deficiency, neurological symptoms can manifest even in the absence of anemia, according to Dr. Brian J. Copeland of University Medical Center New Orleans. Symptoms may develop gradually and may not be immediately recognizable, worsening over time. “Serum vitamin B-12 levels of 200 pg/ml or below would confirm a deficiency; however, measuring homocysteine and methylmalonic acid can also provide insights into vitamin B-12 activity,” said registered dietitian Patricia Kolesa.

A recent study indicated that some older adults exhibited cognitive and neurological issues despite having B-12 levels within the normal range but on the lower end. If concerns arise regarding memory or overall cognitive function, consulting a healthcare professional for a B-12 level assessment can be a crucial step in safeguarding brain health.

“Vitamin B-12 deficiency is common, underrecognized, and relatively easy to diagnose and treat,” stated Dr. Joel Salinas, a clinical associate professor at New York University Grossman School of Medicine. He noted that the neurological and cognitive effects may resemble more serious brain disorders, making B-12 testing a routine part of cognitive decline evaluations.

Identifying Risk Factors for Deficiency

Vitamin B-12 is vital for red blood cell production, and adequate intake through diet is necessary for maintaining healthy levels. The absorption of B-12 requires a protein called intrinsic factor, released by stomach cells. Since the body cannot produce vitamin B-12 independently, insufficient dietary intake or absorption issues can lead to deficiency.

Individuals following vegetarian or vegan diets may be at a heightened risk due to the limited availability of B-12 in plant-based foods. Kolesa highlighted that those with gastrointestinal disorders affecting B-12 absorption, such as Crohn’s disease and celiac disease, as well as individuals with pernicious anemia, a history of bariatric surgery, or heavy alcohol use, are also more susceptible to low B-12 levels. Additionally, older adults face increased risk due to decreased absorption capabilities associated with aging.

Certain medications can also contribute to B-12 deficiency. Long-term use of metformin and acid-suppressing drugs like proton pump inhibitors may lead to decreased B-12 levels, requiring close monitoring.

Addressing Low B-12 Levels

To maintain healthy B-12 levels, individuals should consume a diet rich in B-12 sources, including fish, meat, eggs, dairy products, nutritional yeast, and fortified cereals. For those diagnosed with deficiency, B-12 supplementation may be necessary, either orally or through injections.

Primary care physicians can conduct basic blood tests to assess B-12 levels. “Oral B-12 supplements are effective for most patients, while injectable forms are used in more severe cases,” Salinas explained. Blood testing is crucial in identifying deficiencies before they progress to irreversible stages.

Early intervention in treating B-12 deficiency can lead to the complete resolution of symptoms such as memory loss and mood changes. In the absence of treatment, permanent cognitive impairment may occur, as noted by Copeland. “In the most severe forms, this can progress to frank dementia with behavioral symptoms including psychosis,” he cautioned. This underscores the importance of testing vitamin B-12 levels in individuals experiencing unexplained memory loss.

Awareness of vitamin B-12 deficiency, its symptoms, and associated risks can significantly impact individuals’ health and cognitive wellbeing. Regular screenings and dietary adjustments may help mitigate the risks associated with this often-overlooked deficiency.
