Recognizing Methotrexate Symptoms: When to Seek Urgent Care
Methotrexate is widely recognized as the first-line treatment for rheumatoid arthritis, a condition impacting millions in the UK. This chronic illness causes pain and inflammation in the joints, significantly affecting the quality of life for those diagnosed. While arthritis affects individuals of all ages, including children, it primarily manifests in two forms: osteoarthritis and rheumatoid arthritis. The National Health Service (NHS) reports that osteoarthritis is the most prevalent type in the UK, often developing in individuals in their mid-40s or older.
Understanding Methotrexate and Its Side Effects
Methotrexate serves as a cornerstone therapy for rheumatoid arthritis, but potential side effects warrant careful monitoring. Common symptoms associated with this medication include nausea, loss of appetite, mouth sores, diarrhea, headaches, and hair loss. The medication’s effects extend beyond these symptoms, potentially impacting blood cells and liver function. To ensure safety, patients will undergo regular blood tests, as methotrexate can also infrequently affect the lungs.
The NHS advises that patients may experience heightened sensitivity to sunlight while taking methotrexate, which can leave the skin appearing sunburnt. It is recommended to limit sun exposure and use a high-factor sunscreen with an SPF of 30 or higher.
Serious side effects, while rare, can occur in fewer than 1 in 10,000 patients. Individuals taking methotrexate should be vigilant and seek immediate medical attention by calling 111 if they experience any of the following symptoms:
– Yellowing of the whites of the eyes or skin, indicating possible liver issues
– Persistent cough, chest pain, or difficulty breathing, which may signal lung inflammation
– Swelling in hands, ankles, or feet, alongside changes in urination, suggesting kidney problems
– Fever, muscle aches, or sore throat, which could indicate an infection
– Bleeding gums, blood in urine, vomiting blood, or unexplained bruising, pointing to a possible blood disorder
In the rare event of a serious allergic reaction, patients should call emergency services at 999 if they notice:
– Sudden swelling of the lips, mouth, throat, or tongue
– Difficulty breathing or rapid breathing
– A tight sensation in the throat or trouble swallowing
– Skin discoloration, such as blue, grey, or pale tones
– Sudden confusion, drowsiness, or dizziness
– Loss of consciousness or a child appearing limp and unresponsive
– A rash that is swollen, raised, itchy, blistered, or peeling
Who Should Avoid Methotrexate?
Certain individuals may not be suitable candidates for methotrexate therapy. The NHS recommends consulting with a healthcare provider if you:
– Have a history of allergic reactions to methotrexate or similar medications
– Are pregnant, planning to become pregnant, or breastfeeding
– Suffer from severe liver or kidney disease, or a blood disorder
– Currently have an infection
– Experience mouth ulcers or have a diagnosis of stomach or duodenal ulcers
– Are scheduled to receive a live vaccine, including the live flu vaccine typically administered to children
Additionally, various medications can interact with methotrexate, potentially altering its effectiveness. Patients should inform their doctor if they are taking:
– Non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen
– Antibiotics like co-trimoxazole or trimethoprim
– Diuretics such as indapamide or bendroflumethiazide
– Antiepileptic medications like phenytoin or levetiracetam
– Theophylline for asthma treatment
– Indigestion medications, including omeprazole
– Folic acid or vitamin supplements containing folic acid
